The original state by exam and the most recent state practiced (if different from original.)

If I have just had a Criminal Background Check done, do I have to do another for your Board?

Yes.  The appropriate packet will be mailed to you once your application is received. 

Can a temporary license be extended or reissued?

No.  It is for six (6) months only.

Where do I go to check the status of my application?

Licensure/Listing>Endorsement>Check Endorsement Application Status.
It is the responsibility of the applicant to be sure all required documents are received.

How do I change my address?

Licensure/Listing>Endorsement>Check Endorsement Application Status.
The Endorsement application must be active in order to change an address.  The address change will show in the system the next day.
