If you wish to have our office complete a Case Manager Verification Form, Disability Management Specialist Verification Form or a VISA Screen Form, you will need to submit the form and a fee of $15. You may pay by personal check, money order or credit/debit card. The credit card payment form is available under the "PUBLICATIONS/FORMS" section. Please allow three weeks from the time form and fee are received in the Board office for the form to be completed and mailed to the appropriate agency. The Board mailing address is P.O.
